Go back to basics by shortening the time between scheduled bathroom trips and supervising the puppy pee pad training to going outside (https://wiki.shbf.se/index.php/Managing_The_%27Witching_Hour%27_During_Puppy_Training) closely rather than giving it free run of the home, since unsupervised time is when most missed signals turn into accidents.
